2025 Most Wired Interoperability and Population Health Report
For so long, healthcare provider organizations have poured resources into improving connections for sharing data across networks to improve health outcomes. As the drive for connectivity shifts into a new phase of data utility, maturity becomes more about how organizations turn shared information into better clinical care outcomes.
The 2025 Digital Health Most Wired (DHMW) Interoperability & Population Health segment examines how healthcare organizations exchange data across networks, turn shared information into coordinated workflows, and improve population health outcomes through advanced adoption of person-centric digital health ecosystems.
The Most Wired Interoperability and Population Health Trends Report, sponsored by InterSystems, highlights findings from this segment of the survey and offers providers a wealth of insights to help drive increased maturity.

- Data Governance as a foundation for reliable data exchange – The leading organizations in this area treat data as a discipline. The reward is consistently cleaner data exchange, fewer duplicate records, and higher data completeness. Having formal governance structures is the biggest differentiator.
- National exchange frameworks are widely adopted, but need to be operationalized – TEFCA, FHIR, and other technical foundations have helped build a pathway for scalable data sharing, but as most providers put these frameworks in place, they are now chasing full two-way integration.
- The biggest challenge to data utility is external variability, not internal readiness – Providers working to integrate shared data have found external partner readiness is a weak link in their interoperability maturity journey. This could range from limited EHR interoperability capabilities to misaligned data standards across sources.
